Rhinoplasty Surgery

Nasal Reconstruction Honolulu Hawaii

A rhinoplasty or nose surgery changes the face in a profound way. The nose, being relatively at the center of the face really changes the way your whole face looks. Some of the most common results nose surgery can achieve are: reduce the bump or bridge of the nose, change the tip of the nose, reduce the size of the nose, create a more defined bridge, narrow the nose or nostrils, reduce the flatness of the nose. Rhinoplasty surgery is an outpatient surgery at our facility in one of our state of the art operating rooms. The surgery typically takes 1-3 hours to complete depending on the specific type of rhinoplasty being done.

Rhinoplasty experienced surgeon

Dr. Michael is based in Hawaii and is the Chief of plastic surgery for the entire Infinity Life Centers here in Hawaii, Japan and China. He has over 25 years experience in cosmetic rhinoplasty surgery. He trained in New York City at some of the best cosmetic surgery hospitals in the world including New York University Institute of Plastic and Reconstructive Surgery. Here his experience in ethinic rhinoplasty such as Middle Eastern, Black, Asian was immense. He is one of the few surgeons in Hawaii who uses the closed technique instead of the open technique for most of his rhinoplasties. This is today not learned well by many younger surgeons. The advantage is the closed technique produces mush less swelling so recovery is faster. Of course the open technique is used if needed, but we prefer the closed technique for a quicker recovery.

The Rhinoplasty Procedure in Honolulu, Hawaii

Rhinoplasty can help alleviate birth defects or those caused by injury and also reduce breathing problems. At a purely aesthetic level it can increase and decrease the size of the nose, change the shape of the tip and the bridge also alter the angle of the nose and the upper lip. The procedure is performed by a cosmetic or plastic surgeon and is done under local or general anesthesia. Rhinoplasty usually takes an hour or two to perform and the time varies according to the amount of surgery required. The skin is first separated from the bone and it is sculpted to provide the desired appearance, thereafter the skin is put back in place. The rhinoplasty surgery may be performed by making incisions inside the nose; thus leaving no apparent scarring. While an ‘open procedure’ would involve making a small incision in the columella or the strip of tissues that separates the nostrils. Once the surgery is completed a splint is used to support the nose and to maintain its new shape. Also nasal packs may be applied in the nostrils to support the septum, the wall that divides the air passages.

Recovering from Rhinoplasty

After the rhinoplasty the nose would feel swollen and ache and the area around the eyes may also be sore. Medication and cold compression may be used to ease the discomfort. When lying down, the patient is advised to keep the head at an elevated position higher than the rest of the body. A little bleeding is common initially after the surgery and the surgeon would advise not to blow the nose. In about 10 days time the stitches are removed and the patient can go back to his or her normal routine. However strenuous activity and exercise that increase blood pressure must be avoided for several weeks, until the surgeon advises. It would take about a year for the nose to recover completely from the procedure and it is important that the patient adheres to all the recommendations and precautions that the surgeon advises.

Rhinoplasty (nose job), Nose Implants, Asian Rhinoplasty, & Nose Reshaping

A nose that fits the face is an important concept in rhinoplasty also known as nose reshaping. This is one of the characteristic features of the face and can dominate other features. To reshape the nose can have a dramatic effect on the eyes, the chin and the overall facial appearance. Here at Dr. Pasquale’s clinic in Honolulu Hawaii we have experience in Asian Rhinoplasty, Caucasian Rhinoplasty, African American Rhinoplasty as well as the many ethnic noses around the world.

Closed Rhinoplasty vs Open Rhinoplasty

The nose reshaping done by Dr. Pasquale is usually performed as a closed rhinoplasty. This technique is all done with the cuts inside the nose as opposed to the open rhinoplasty techniques used by many surgeons today. The open technique which leaves external scars. We believe plastic surgery such as rhinoplasty should be done with very little noticeable scarring.

What makes Dr. Pasquale’s Clinic in Hawaii different is that we have over 25 years experience with Rhinoplasty on Asians, Caucasians and African Americans, yet each rhinoplasty we perform is unique. We do not follow a Rhinoplasty formula or text book approach, but take special care to use a combination of skill and technique to give you the nose you want.

Nose Reshaping in Hawaii

They say that one shouldn’t judge a book by its cover but the fact of the matter is that appearances are very important in the world that we live in today. While most of us are generally content with the way we appear there is always room for improvement and a little difference and alteration can go a long way.

The nose reshaping procedure or rhinoplasty is performed by a plastic or cosmetic surgeon and takes about 1-2 hours. The time of the surgery may vary according to the complications of the procedure. Nose reshaping may be done under local or general anesthesia.

The minimum age for nose reshaping is suggested as being more than 15 years, when the nose has grown fully (in boys the age is considered a little more) and the upper age limit isn’t fixed. What is important is that the individual be of good physical and mental health.

The incision for the surgery may be made inside the nostrils or in case of an ‘open technique it is made at the base of the nose. The open technique is essential in procedures where the nostrils are to be narrowed. The skin is removed from the bone and once the bone and cartilage are desirably reshaped it is placed back. Once the surgery is completed, a splint is used to support the nose and to maintain the new shape.

Cosmetic Nasal Surgery (Nose Job)

The nose is perhaps one of the most prominent features of the human face. It adds character and balance to it. Have you looked in the mirror ever so often, wishing that you could alter the way your nose looks? Cosmetic nasal surgery provides the perfect solution. Not only can this procedure take care of birth defects, injury and breathing problems but also reshape the nose to look more aesthetically pleasing. Cosmetic nasal surgery provides individuals to reshape their nose by increasing or reducing its size, reshaping the tip, reducing the size of the nostrils or changing the angle between the nose and the upper pp. Cosmetic nasal surgery is performed by a cosmetic or plastic surgeon and it is vital that you select a skilled and competent surgeon.

While cosmetic nasal surgery is available in most parts of the country, Honolulu, Hawaii is the top destination for people looking to have rhinoplasty done. This is basically because of the top grade surgical and medical service available here. Also the beautiful setting of this tropical paradise provides a great place to recover after the cosmetic nasal surgery. Dr. Michael Pasquale in Honolulu, Hawaii is one of the few surgeons in the world who has completed his residency in plastic surgery and gone ahead to do a 2-year training course in the sub-specialty of cosmetic surgery under the country’s top plastic and cosmetic surgeons at NYU Downtown Hospital in Manhattan.

About Cosmetic Nasal Surgery (Rhinoplasty)

An ideal candidate for cosmetic nasal surgery (Rhinoplasty) should not be of less than 15 years so that growth of the nose has taken place properly before the surgery (in boys it takes a while longer). There is no upper age limit for the cosmetic nasal surgery as long as the individual is in good mental and physical health.

A rhinoplasty takes about 1-2 hours and may take longer depending on the extent of surgery required. Cosmetic nasal surgery is performed under local or general anesthesia.

Rhinoplasty involves removing the skin from the bone and cartilage and reshaping the latter as desired. Thereafter the skin is replaced. The incisions for the surgery may be made from inside the nostrils or in an ‘open technique’ at the base of the nose. The ‘open technique’ is required in particular if the nostrils need to be narrowed. After the surgery a splint is used to support the nose and maintain its shape.

Rhinoplasty Scarring

Scarring from rhinoplasty surgery is minimal and would be virtually invisible in a few months.

Initially after the cosmetic nasal surgery (rhinoplasty) the nose and surrounding area feels swollen and sore and the surgeon may prescribe medication for the same. Cold compressions are also useful in reducing the discomfort. The patient is advised to keep his or her head elevated while lying down and the swelling reduces after the stitches are removed in about 10-14 days. Thereafter the individual can return to his or her normal routine but needs to be careful and adhere to the precautions suggested by the surgeon. In particular activities that increase blood pressure should be avoided. It takes about a year for the patient to fully recover from cosmetic nasal surgery.

While risk of complications with cosmetic nasal surgery or rhinoplasty is minimal there is always a chance of infection, complications due to a reaction to anesthesia, nosebleed, etc. However, selecting the right surgeon and adhering to all the post-operative care advice can dramatically reduce these.

Cosmetic Nasal Surgery Complications

Most risks of complications can be eliminated by selecting a good surgeon who is not only well qualified but is also experienced and skilled. Also important is the post operative care that is taken by the patient and it is vital that he or she adheres to all the precautions and recommendations made by the cosmetic or plastic surgeon.

  • Post operative bleeding: While some amount of post operative bleeding is expected, it may be increased due to the intake of medication such as Aspirin, ibuprofen and certain nutritional supplements such as vitamin E. Intranasal packing can be used to avoid excessive bleeding.
  • Complications due to use of anesthesia: As in any surgery where anesthesia is required there may be some unexpected reactions to the anesthesia. These can include cardiac and pulmonary complications.
  • Taking proper postoperative care can drastically reduce a chance of infection developing.
  • One of the functions of the sinuses is to affect resonance and thus nasal surgery may result in change in the voice.
  • In some cases the sense of smell may also be affected by nasal surgery.
  • There may be redness and bloods vessels may be prominently visible and also pigmentation in the region may occur.
  • The nasal passage may be disturbed in rare cases leading to disturbance in the air path.
  • Due to under or over correction the nasal tip may be effected and similarly under correction of the bone may lead to a less than desirable appearance while over correction may lead to lack of support for the nose.
  • Asymmetrical displacement of structure may result in distortion of vision and the appearance would be less than satisfactory.
  • In extremely rare case intra-cranial complications may occur. This happens when the base of the skull which forms the roof of the ethmoid and sphenoid sinuses is disturbed leading to a leak in cerebrospinal fluid. Also intra-orbital complications are seen infrequently, wherein the orbit is located close to the paranasal sinuses and excessive bleeding may occur into the orbit. This can be corrected at the time of surgery itself.

You may also notice some oxygen being given until your fully awake.

Recovery really goes through two phases. The first is in the first 5 days to 3 weeks after surgery when the swelling starts to go down. If you have bruising that will disappear during this time. The second phase is when the nose starts to take its permanent new shape, meaning there will still be subtle changes taking place up until a year. Your surgeon most likely will tell you to be patient as it can be a long time before the final artistry is seen. On average most patients can return to work in ten days or less even for more complicated procedures.

Common Aspects of Rhinoplasty Recovery

  • Swelling this can resolve very quickly in many people, it can be at the tip or the bridge or both. The nose may not appear to have changed much during this time due to swelling.Swelling is normal and will resolve with time , taping the tip and the splint helps resolve this.
  • Bruising may be seen which is the black and blue under the eyes. This in the recovery period can be covered with makeup. In many cases no bruising is seen.
  • Pain in rhinoplasty typically can be taken care of with medication. Most people do not complain that it is uncomfortable yet again that will depend on each individual and the exact procedure that was done. We always make sure you have been give adequate medication so you are very comfortable.
  • Back to work is usually for more complex cases in 10 days for simple cases in 5 days, but you need to ask your surgeon.

There is a lot of material on the Internet about recovery for rhinoplasty you can read about. Some of this information is very good and some may not be true in your case. This will be depending on the type of rhinoplasty you have had. So its always best to speak to your individual surgeon who performed your surgery.

Our surgeons at the infinity life center who perform rhinoplasty will always be available to answer your questions and along with the staff take special pride in guiding your recovery so just ask. Your patient coordinator will always do all they can do to make sure your comfort and safety is our number one concern.

Nasal Surgery with Silicone Implants

There is nothing that adds more character to a face than a well-shaped and balanced nose. While some people are born with a nose that needs no alteration most of us desire to reshapes our noses in order to achieve a more pleasing appearance. Augmentation of the nose by nasal surgery is also known as rhinoplasty and is a popular plastic surgery. The risk involved in the surgery is minimal and the final effect helps accentuate the individual natural features. The different types of materials used for augmentation of the nose include alloplastic, homologous and autologous. Alloplastic includes synthetic materials of which silicone is one of the most popular.

The ideal material for augmentation of the nose should have the following features:

  • Tolerance for the material by the host.
  • Stability of the material.
  • Malleability
  • The material should not be prone to discoloration.
  • It should easy to handle.
  • The material should be easily available and in adequate amounts.

Silicone implants in nasal surgery meet most of the mentioned criterion. Silicone implants are easily available and in adequate supply, they are easy to handle and shape, resistant to warping and resorption and have no donor-site morbidity. However silicone implants need to be handled carefully in order to reduce a chance of extrusion and infection. Silicone implants are found to be particularly effective in the upper third part of the nose as there is less chance of the implants moving here and lesser proximity to nasal lumen.

Before the surgery the surgeon will physically assess the area and evaluate the bone and tissue of the nose. The minimum age for the procedure is about 15 years when the nose has grown fully and there is no upper age limit. It is vital that the candidate is of good physical and mental health.

The surgery with silicone implants in nasal surgery is performed under local or general anesthesia and usually lasts for over an hour. An incision is made from the inside of the nose. The nose is dissected in order to create space for the silicone implants. Thereafter soft medical grade silicone implants are inserted to increase the height and shape of the nose. The nasal tip is defined properly. Self-dissolving stitches are usually used in silicone implants with nasal surgery.

Recovery from Nasal Surgery with Silicone Implants

A splint is placed to provide support to the nose and maintain the created shape. The nose and surrounding area would feel swollen and sore. Besides cold compressions the surgeon would probably prescribe medication to ease the discomfort. It is advised that the head be kept elevated even while lying down. The implant stabilizes with the nasal joint in a week and after that one can return to one’s normal routine. It is however advised that strenuous activities and exercise be avoided until advised by the surgeon.

Rhinoplasty Augmentation

Rhinoplasty augmentation is usually not required in primary rhinoplasty but it is resorted to when reduction of the nose has been done too aggressively, to correct deviations and asymmetry in the nose, increase the height of the nose in order to create a balanced relation with the tip, to create a narrowed look of the nose without changing the airway, to correct deformities that may be congenital or due to injury and to provide an overall balanced and symmetrical look to the nose and face. Rhinoplasty augmentation basically means to increase the shape of the nose by using various types of material. Augmentation rhinoplasty is available in Honolulu, Hawaii, which is the popular choice of individuals looking to have cosmetic and plastic surgery done. Honolulu, Hawaii is the proud home to many world-class surgeons such as Dr. Michael Pasquale, who has done his residency in plastic surgery and also earned a 2-year training certificate in the sub-specialty of cosmetic surgery from NYU Downtown, Manhattan. Dr. Pasquale is also an osteopathic physician and surgeon (D.O.). At his clinic it is ensured that clients understand the various techniques and types of materials available for Rhinoplasty augmentation. Furthermore patients are made aware of the advantages and disadvantages of the different materials that may be used for Rhinoplasty augmentation and also the risk of complications associated with this plastic surgery.

Silicone Nasal Implants (Alloplasts)

Alloplasts or synthetic implants such as silicone, polytetrafluoroethylene and porous polyethylene. Some of the advantages of using these materials include easy availability, malleability, resistance to warping and resorption and no need to source the material from the patient. However an increased rate of infection is found when alloplast implants are used as compared to autologous implants.
Homologous materials sourced from cadavers.

Cartilage or Rib Nasal Implants (Autologous Implants)

Autologous implants that are obtained from the patient’s own body are found to have the lowest incidence of infection and are stable. They have an inherent ability to adapt to the relocation to the host bed. Autologous materials include cartilage (septal, auricular and rib) and bone (ossified bone), these are easy to harvest and graft and easy availability of material makes the Rhinoplasty augmentation simpler.

The exact procedure for the Rhinoplasty augmentation will be determined by the cause of the surgery and an assessment of the extent of surgery is made from a careful physical examination of the nasal area. The surgery is performed under anesthesia by a cosmetic or plastic surgeon. And it is recommended that you select a surgeon who has experience and expertise to carry out the procedure. After the surgery a splint is used to support the nose and to maintain the shape created by Rhinoplasty augmentation. Post-operative Care for Rhinoplasty augmentation Post-operative care for Rhinoplasty augmentation is similar to that for primary rhinoplasty. The nose and the neighboring area will remain swollen for a while and feel sore. Medication and cold compression can reduce the discomfort. The swelling will reduce after stitches are taken out in about 10-14 days. It is essential to keep the head elevated when lying down. While one can return to normal routine in 2 weeks precautions suggested by the surgeon must be adhered to.

Asian Rhinoplasty in Hawaii

Asian Rhinoplasty in Hawaii is very common with a poplulation of 70% Japanese , Chinese , Filipino and Korean and only about 30% other such as Caucasian. An Asian Rhinoplasty is done somewhat different than other races. The reason is that typically although not always, there is a very low bridge and less cartilege than other ethicities.

Rhinoplasty Surgeons in Hawaii

Thus a lot of Asian nose work is basically creating new tissues such as a higher bridge on the nose or a more defined nasal tip. This is completeltly different than for example a middle eastern nose where most of the surgeon’s time is taking away some of the cartilage. Also one wants to shape the nose differently for Asians than others. A pointy tip if over done in Asian looks very odd for example so the surgeon should have a good concept of Asian beauty. Then must execute this with an artistic eye. Hawaii plastic surgeons are well versed in Asian aesthetics as mentioned Asians are in the majority.

We Teach Asian Rhinoplasty in Korea, Japan & China

Our Chief of Plastic Surgery Dr. Michael has been a recognized faculty member and instructor in China , Japan and Korea. He lectures there on Rhinoplasty and other topics regarding Asian beauty. Additionally he commonly has visiting students from Asia to come to our clinic here. So rest assured that we have the expertise at the Infinity Life Center for Asian Rhinoplasty.\

Repair & reconstruction of previous asian rhinoplasty

We have special expertise in reconstruction of previous unfortunate outcomes of badly damaged Asian noses. This requires special expertise and experience. Sometimes unfortunately only moderate improvement can be carried out. One thing for sure is we will provide you with options and what we feel is best to correct the Asian rhinoplasty case.

At Infinity Life Center we have not just one resource to draw from, but have many consultants who we can help if you have a very difficult problem. We do not hesitate to provide you with care from our consultants when needed as we want you to have the best care we can possibly provide.

Our initial consultations are with out charge for cosmetic surgery. We have Asian speaking patient coordinators who are fluent in Japanese , Chinese and Korean available. Many of our patients come from off island so it is possible to have a virtual consultation through the internet.

Asian Rhinoplasty

Asian noses present unique challenges to the plastic surgeon. There is often little to no bridge present and a smaller underlying bone structure on which to build. During Asian rhinoplasty, Dr. Pasquale will raise and narrow the bridge of the nose, generally using grafted cartilage from the ear, as well as narrow the nasal width from nostril to nostril.

Depending on the look desired, he may also reshape or lift the tip of the nose. The results are a strong, well-defined profile and a nose that is in harmony with the rest of the face. Like Asian eyelids, Asian rhinoplasty requires a special understanding of the Asian concept of beauty in order to be successful. Dr. Pasquale has extensive experience working with Asian clients and would be happy to discuss your goals with you.
